Background: Despite a healthy lifestyle, endurance athletes can develop obstructive coronary disease and require appropriate evaluation,
medical and interventional therapies.
Case: A 58-year-old healthy male presented to the emergency department after a witnessed cardiac arrest while exercising at a gym.
He had a shockable rhythm and was successfully resuscitated. The patient was an endurance athlete, exercising regularly, sometimes
twice day. In the week prior to presentation, he had been experiencing exertional chest pressure that resolved with rest. Exam was
unremarkable, he was physically fit and well appearing. Electrocardiogram showed no ischemic changes. Echocardiography revealed
